CHSI leadership would like to collect information about the experience of participants. An invitation to complete a survey will be sent by email to each student. Students will click on a link in the email and enter a software program which collects data, aggregates the information, and protects privacy. Data will be collected by an External Evaluator, Dr. Judith Sunderman. For additional information, feel free to contact her at [email protected] or 217-232-8304.

The survey will ask students about their experience with CHSI and collect information on their beliefs about entrepreneurism and innovation.  The collected information will be shared with CHSI leadership and will be used to improve CHSI.

Participation is voluntary. Participation or nonparticipation will not impact your opportunities with CHSI or any CHSI partners. Please indicate your agreement by filling out the form below.
